Community Support Program (CSPs) provide an array of services delivered by a community based, mobile, multidisciplinary team of paraprofessionals. These programs provide essential services to Members with a long-standing history of a psychiatric or substance abuse disorder and to their families, or to Members who are at varying degrees of increased medical risk, or functioning in the home/community setting.
- Assisting members in enhancing their daily living skills

CBHI IN-HOME THERAPY CLINICIAN
Job Title: CBHI IN-HOME THERAPY CLINICIAN

Role Description
CBHI In-Home Therapy Clinician: Deliver exceptional professional and therapeutic counseling services to children and their families. Actively participate in and facilitate wrap-around services that foster holistic support for our clients.
- Provide home-based individual therapy, family therapy, and case management services to children, adolescents, and families.
- Collaborate seamlessly with case managers, physicians, and nurses to develop well-coordinated treatment strategies tailored to the unique needs of each client and their family, while also engaging with outside agencies involved in the client’s care.
- Offer compassionate guidance and support to family members, helping them understand and navigate the complexities of the client’s situation while fostering a supportive environment.
- Conduct impactful individual and family counseling sessions aimed at helping clients overcome challenges such as mental and physical illnesses, poverty, unemployment, and experiences of abuse.
- Enhance your professional expertise by consulting with licensed practitioners in weekly or bi-weekly individual or group collaborative sessions.
- Thoroughly interview clients, review records, conduct comprehensive assessments, and collaborate with other professionals to accurately evaluate the mental and physical conditions of clients. Regularly update and modify client treatment records as needed.
- Monitor, assess, and document client progress toward treatment goals.
- Attend client meetings as required and coordinate efforts with external stakeholders, such as the Department of Children and Families (DCF), the Department of Youth Services (DYS), or Juvenile Probation, to ensure collaborative support.
- Connect clients and their families with vital community resources for housing or treatment, facilitating their recovery journey from mental or physical illnesses, and follow up diligently to ensure service effectiveness.

Mental Health Out-Patient Therapy Clinician
Job Title: Mental Health Out-Patient
Department: Worcester

Primary Functions:
Out-patient services provide individual, family, couples, and group counseling to children and adults. Services include home-based and center-based treatment.
Tasks:
•Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, and providing support;
•Collaborate with case managers, physicians, and nurses to plan and coordinate treatment based on client and family needs; and also with outside agencies involved in the treatment of the client i.e.: ICC workers;
•Counsel and aid family members to assist them in understanding, dealing with, and supporting the client;
•Counsel clients in individual and family sessions to assist them in dealing with mental and physical illness, poverty, unemployment, or physical abuse;
•Interview clients, review records, conduct assessments, and confer with other professionals to evaluate the mental or physical condition of client or patient;
•Modify treatment plans and CANS on a quarterly basis through virtual gateway and the company EMR system;
•Document sessions or any contacts made in the company’s EMR;
•Monitor, evaluate, and record client progress with respect to treatment goals;
•Attend school based meeting as needed, coordinate with outside agencies such as DCF, DYS, or Juvenile Probation;
• Refer client, or family to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy.

Therapeutic Mentor
Job Title: Therapeutic Mentor
Department: Worcester

Therapeutic Mentor offers structured, one-to-one, strength based support services between a therapeutic mentor and a youth for the purpose of addressing daily living, social, and communication needs. Therapeutic Mentoring services include supporting, coaching, and training the youth in age-appropriate behaviors, interpersonal communication, problem –solving and conflict resolution, and relating appropriately to other children and adolescents, as well as adults, in recreational and social activities.
- Intensive Therapeutic Mentoring which includes establishing relationships with youth and engaging them in skill building activities: (e.g. increasing impulse control, building socialization skills, delayed gratification, exploring academic pursuits and enhance self -esteem.)
- Identify activities for each child assigned and locate appropriate activities in the community.
- Develop and implement lesson plans utilizing the clinician’s treatment plan.

Therapeutic Training and Support
Job Title: Therapeutic Training and Support

Therapeutic Training and Support (TT&S) is a service provided by a qualified paraprofessional working under the supervision of a clinician to support implementation of the clinician’s treatment plan to assist the youth and family in achieving the goals of that plan. The paraprofessional assist the clinician in implementing the therapeutic objectives of the treatment plan designed to address the youth’s mental health, behavioral and emotional needs. This service includes teaching the youth to understand, direct, interpret, manage, and control feelings and emotional responses to situations and to assist family to address the youth’s emotional and mental health needs. Phone contact and consultations are provided as part of the intervention.

Substance Abuse Counselor
Job Title: Substance Abuse Counselor

Brief description
The position of substance abuse counselor consists of assessing and treating individuals with substance abuse problems, including abuse of alcohol, tobacco, and other drugs.
Primary Functions:
- Collaborate with counselors, physicians, and nurses to plan and coordinate treatment,
- Counsel and aid family members to assist them in understanding, dealing with, and supporting the client or patient;
- Counsel clients in individual and group sessions to assist them in dealing with substance abuse illness;
- Educate clients and community members about substance abuse;
- Increase counseling knowledge by reviewing current literature, conducting research, and attending seminars, training workshops, or classes;
- Interview clients, review records, conduct assessments, and confer with other professionals to evaluate the client;
- Modify treatment plans according to changes in client status;
- Monitor, evaluate, and record client progress with respect to treatment goals;
- Refer client, or family to community resources for housing or treatment to assist in recovery, following through to ensure service efficacy.
